What is TMJ?

The temporomandibular joint (TMJ) is the hinge that connects the jaw to the skull. It allows for essential functions like chewing, speaking and swallowing. TMJ disorders occur when this joint is not functioning correctly, leading to pain, discomfort and difficulty moving the jaw.

The Clinical Oral Investigations Journal published a study in 2021 claiming that the overall prevalence of TMJ disorders was approximately 31% for adults/elderly and 11% for children/adolescents. Apparently, many people suffer from this condition without knowing it.

What is Sleep Bruxism?

Sleep bruxism is the involuntary grinding or clenching of teeth during sleep. It can occur with or without TMJ disorders, although they are often seen together. In 2023, the American Family Physician® magazine published an article that revealed between 5% and 12% of the population have sleep bruxism and TMJ issues.

People who grind their teeth may not be aware of it until they experience symptoms such as:

  • Worn-down teeth
  • Tooth sensitivity or pain
  • Jaw soreness or stiffness, especially in the morning
  • Headaches
  • Disruption of sleep patterns

The Connection Between TMJ and Sleep Bruxism

Bruxism can lead to or exacerbate TMJ disorders due to the increased pressure and strain placed on the jaw joint and surrounding muscles. As individuals grind their teeth, they may inadvertently cause inflammation and irritation to their TMJ, leading to a cycle of pain and discomfort. Conversely, individuals with TMJ disorders may inadvertently clench their teeth to alleviate discomfort, further contributing to bruxism.

Treatment Options for TMJ Disorders and Sleep Bruxism

Addressing both TMJ disorders and sleep bruxism often involves a multifaceted approach:

  1. Oral Appliances: Custom-fitted mouthguards or splints can help alleviate the pressure on the TMJ by providing a barrier to grinding at night.
  2. Physical Therapy: Exercises to strengthen jaw and neck muscles can help improve function and reduce pain. Therapists may also use modalities such as ultrasound and heat therapy.
  3. Medication: Over-the-counter pain relievers or prescription medications can help manage pain and inflammation. Muscle relaxants may also be beneficial for reducing tension in the jaw.
  4. Stress Management: Since stress often contributes to grinding and TMJ discomfort, techniques such as yoga, meditation and deep breathing exercises can be effective.
  5. Dental Treatments: In more severe cases, dental work to correct bite problems or misalignment may be necessary.
  6. Surgical Interventions: In rare cases, surgery may be needed to correct structural issues within the joint.
